The primary goal of outreach support is to bridge the gap between individuals who require care and the available care services. We aim to identify and engage individuals who are isolated, vulnerable, or facing specific challenges that hinder their access to healthcare, social services, or other essential resources thereby improving the overall well-being and quality of life of individuals by extending care beyond institutional settings and proactively engaging with those who may face barriers to accessing services.
This approach is particularly beneficial for marginalised populations, such as the elderly, individuals with disabilities, mental health issues, or substance abuse problems, as well as those living in remote or underserved areas.
