End of life or hospice care provides comprehensive support to individuals with a focus on providing comfort, dignity, and quality of life to individuals who are in the final stages of a life-limiting illness or nearing the end of their lives.
The goal of this type of care is to ensure that patients experience a peaceful and comfortable transition while addressing their physical, emotional, and spiritual needs while also providing support to families throughout the grieving process.
The aim is to enhance the end of life experience for both the patient and their loved ones. This involves a multidisciplinary team of healthcare professionals and volunteers, who work together to meet the unique needs of each individual and their family.
Below is a detailed description of end of life or hospice care services:
Philosophy of Care: This care operates on the principle that every individual deserves to spend their final days in a comfortable and supportive environment, surrounded by their loved ones. Our focus is on quality of life rather than curative treatment, with an emphasis on managing pain and symptoms, promoting emotional well-being, and facilitating spiritual and cultural practices.
Multidisciplinary Team: We collaborate with doctors, nurses, social workers, counsellors, chaplains, and trained volunteers to address medical, emotional, social, and spiritual aspects of care of patients and their families.
Pain and Symptom Management: One of the primary goals of hospice care is to alleviate pain and manage distressing symptoms. We closely monitor and assess the patient's symptoms, following medicines adjustments medications and treatments as necessary from doctors to provide optimal comfort. This involves the use of pain medications, physical therapies, complementary therapies, and other interventions.
Emotional and Psychosocial Support: Hospice care recognises the emotional and psychosocial impact of a life-limiting illness on both the patient and their loved ones. In liaison with social workers, counsellors, and other professionals we offer emotional support, counselling, and guidance to patients and families to help them navigate complex emotions and make informed decisions about their care.
Spiritual Care: Recognising the importance of spirituality and individual beliefs, we provide spiritual support through chaplains or other religious leaders. They offer guidance, perform religious rituals or ceremonies, and provide comfort based on the patient's spiritual or religious preferences.
Assistance with Activities of Daily Living (ADLs): We assist with ADLs, such as bathing, dressing, grooming, and mobility, as necessary. Our staff provide this support, helping patients maintain their personal hygiene and independence for as long as possible.
Respite Care: We offer respite care to provide temporary relief to family caregivers who may need a break from their caregiving responsibilities. This allows them to rest, address personal matters, or simply take time for self-care while their loved one is under the care of trained professionals.
Bereavement Support: We continue to support families even after the patient has passed away. Bereavement support services, such as counselling or support groups offer to help family members navigate their grief and adjust to life without their loved one for an extended period to ensure ongoing support.
In-Home or Inpatient Care: We provide services in various settings, including the patient's home, or assisted living facilities. The choice of location depends on the patient's needs, preferences, and the level of care required. The aim is to create a comfortable and familiar environment where the patient feels most at ease.
Care Planning and Advance Directives: We encourage open communication and shared decision-making. Our care team collaborates with patients and families to create personalised care plans that align with the individual's goals, values, and preferences. This involves discussions about advance directives, such as living wills or do not resuscitate orders, to ensure that the patient's wishes are respected.
